Clinic Visit

Your time is valuable, and we need to influence your visit as thoroughly and briefly as we can. Having data early will encourage this procedure from the minute you are welcomed by the staff.

Included is a list of items to convey to your office visit or for your hospital stay, as well as phone numbers you may use to contact the division. Your health is important, and questions arise from time to time; that is why we have clinicians to respond to your inquiry.

When visiting the Heart and Vascular Associates clinic, please be sure to bring the following:

New Patient Form
HIPAA Privacy Authorization Form
Scribe Consent Form

Instructions: Please complete the left side and the entire page.

In addition, please bring:

• Insurance Card(s)
• Photo ID (i.e. driver’s license)
• Referral – if your insurance requires one.
• Copay – if your insurance requires one.
• Recent cardiac testing and lab
• Current medications (in their container)

For HIPAA compliance, patient-related documents should not be sent via email. Please note that any documents containing protected health information (PHI) submitted through email will not be accepted, as this method is not considered secure or compliant with HIPAA regulations.
